Talk to your Teacher-

It's that simple!  It shows you care, concerned about your performance and it's notifying the teacher that additional reinforcement is needed.  Perhaps your teacher can provide some insight into what you could improve upon to make the curriculum easier to understand, ease test taking anxiety with more focused preparation, discuss homework and/or missing assignments and perhaps arrange after school help!
