WHAT NEEDS TO BE DONE TO PARTICIPATE:

• Obtain the Westhill Athletic Handbook, Required Medical Paperwork Packet, Permission for Medical Treatment Index Card and Athlete/Parent/Guardian Pledge Form.   These forms can be downloaded from the Westhill High School Website (with the exception of the index card), or picked up from the Main Office, School Nurse’s Office or the Athletic Director’s Office.  Student-Athlete and parent/guardian need to read the handbook together and go over all of this paperwork.
• Make an appointment with a physician for a physical exam.  Bring the medical paperwork to the appointment so that the physician can fill out the necessary forms.
• Parent/Guardian completes the Permission for Medical Treatment Index Card, Sports Participation Permission Form and the Sports Medical History Form.
• Bring all of these required forms to the School Nurse for approval prior to the first day of practice.
• The student-athlete and parent/guardian sign the Athlete/Parent/Guardian Pledge Form.
• The student-athlete should arrive on time for the first day of try-outs with an “Approved for Play” stamped index card from the School Nurse and a signed Athlete/Parent/Guardian Pledge Form.
