WHS Attendance Appeal Form 2022-2023 Semester 2 and Full Year Courses ONLY

Please Note: Documentation to excuse these absences must have been submitted at the appropriate time (within 10 days of the absence) for an appeal to be considered.  Please note that no appeal will be accepted if you have 5 unexcused absences for a semester 2 course and 10 unexcused absence for a Full Year Class. Aside from the exemptions in the Board of Education Policy and Regulations on attendance, the only reason for filing this appeal is for extraordinary circumstances not specifically outlined. (Please attach any documentation to support your request for an appeal):

Tenga en cuenta que la documentación de las ausencias deben haber sido hechas en el tiempo que se advirtió. Además no se considerarán apelables aquellos estudiantes que tengan 5 o más ausencias del semestre 2 y 10 de ano completo y que no sean apelables. La única razón por la que se tomaría en cuenta sería por causas extremas. (Por favor adjunte cualquier documentación que ayude para la apelacion).

At the end, please upload written documentation as per the policy (for example: a doctor's note, court summons/appointment, official college visit invitation/email, or parent/guardian note). NOTE: YOU CAN ONLY UPLOAD ONE NOTE/ABSENCE AT A TIME. You can take a picture of your documentation (with your smart phone) and upload the picture. Please make sure the picture is CLEAR and all parts of your documentation are clearly visible, or it cannot be considered as an excuse or exemption.
