Absence Documentation
Students: Please fill in the following form when documenting your absences. You will need to use your school-issued Stamford Public Schools Gmail Account. To ensure that your absence is documented properly, please upload written documentation as per the attendance policy above (for example: a doctor's note, court summons/appointment, parent/guardian note, etc.) and that the upload is clearly visible. If the note is not for a reason listed in the attendance policy, it cannot be considered as an excuse or exemption.
Estudiantes: Por favor complete el siguiente formulario cuando documente sus ausencias. Deberá utilizar la cuenta de Gmail de las Escuelas Públicas de Stamford emitida por su escuela. Para garantizar que su ausencia se documente adecuadamente, cargue documentación escrita según la política de asistencia anterior (por ejemplo: una nota del médico, citación/cita judicial, nota del padre/tutor, etc.) y que la carga sea claramente visible. Attendance Policy Highlights:
The administration expects that students will attend every class daily. Students, Parents/ Guardians, Teachers, School Counselors, and Administrators share responsibility for student attendance.
- Students must attend class and keep track of any absences. They must make up any work missed during absences.
- Parents/Guardians must provide the school with updated phone numbers and must support the emphasis on regular daily attendance.
- Teachers must record attendance daily.
- School Counselors must inform administrators of absence patterns.
- Administrators must enforce the attendance policy and regulation.

The State of Connecticut requires that all student absences be documented.

Documentation, if submitted within 10 days of the absence, will permit students to make up work per teacher expectations. Undocumented absences will result in additional intervention.
Tardiness:
Daily Attendance Codes:
● (A)= Unexcused Absence
● (ET) = Excused Tardy
● (TT) = Unexcused Tardy for less than half of block
● (AT) = Absence Tardy for those who arrive after 50% of the class

- BX - Bereavement
- CX - Court Appearance
- EA - Early Dismissal
- FX - School Related Activity/Field Trip
- LX - Long-Term Illness (doctor’s note)
- MA - Non-Emergency Medical Visit
- MHX - Mental Health Day (2 per year)
- RX - Religious holiday
- SA - Short-Term Illness (parent/guardian note)
- SX - Suspension
- STX - Short-Term Illness (doctor’s note)
- VAX - College Visit/Orientation

THE ATTENDANCE THRESHOLD
When a student is earning a passing grade for the quarter in a class:
- More than 3 Unexcused Absences in that class during the quarter = Quarter Grade Adjusted to 60 (D-)
- This applies by class. T
- he attendance threshold is based on the number of unexcused absences in that specific course.
WHAT HAPPENS WHEN A STUDENT EXCEEDS THE THRESHOLD?
- A student who exceeds the 3-unexcused-absence threshold and has their quarter grade adjusted to 60 (D-) may choose to participate in a GRADE RESTORATION MEETING.
During the meeting, the Attendance Committee will review:
RESTORING THE STUDENT'S GRADE
Students who attend the Grade Restoration Meeting will:

Review the attendance policy
- Discuss the impact of their attendance
- Identify ways to prevent recurrence
- Sign a Grade Restoration Contract
IF PREVIOUSLY EARNED QUARTER GRADES ARE RESTORED
- Grade restoration applies to ALL classes in which the student exceeded the unexcused-absence threshold for that quarter.
‼ IMPORTANT: STUDENTS GET ONE GRADE RESTORATION OPPORTUNITY PER YEAR !!

Grade Restoration is offered ONE TIME per school year.

Students should understand that this is an opportunity for restoration and accountability, not an automatic reset of the attendance requirement.
